Verminderung des Harnröhrentraumas durch eine Boutonnière bei der transurethralen Prostata-Resektion, ein scheinbarer Widerspruch

Zusammenfassung:

Das Harnröhrentrauma bei TUR-P läßt sich bei bestimmten Indikationen durch die Anlage einer Boutonnière deutlich vermindern. Insbesondere läßt sich mit diesem Verfahren die Rate der postresektionelle Harnröhrenstrikturen verringern. Als Boutonnière wird eine Stichinzision zwischen Skrotalansatz und After bezeichnet, über welche der Resektionsschaft nach Eröffnung der Harnröhre in die Blase vorgeschoben wird. Bei 206 von 3914 transurethralen Prostata-Resektionen wurde dieses Verfahren seit 1988 durchgeführt. 162 Patienten konnten nachuntersucht werden, 44 Patienten waren zum Zeitpunkt der Untersuchung verstorben, 154 Patienten waren mit dem Ergebnis der Operation zufrieden, 2 Patienten entwickelten eine Striktur.

Abstract

TUR-P across a Boutonnière is a qualified procedure to reduce postoperative complications of TUR-P, especially to avoid strictures of the urethra. A Boutonnière is an incision between scrotum and anus, through which the resectoscope is inserted into the bladder. Since 1988, 206 patients were operated with this procedure. Of these, 162 patients could be examined in a follow-up study. A total of 44 patients had already died at the time of examination, 154 patients were content with the result of the operation, 2 patients had developed a stricture.
