Thrombose der Vena cava inferior nach stumpfem Bauchtrauma als Ursache retroperitonealer Raumforderungen mit Abflußbehinderung der Niere

Zusammenfassung:

Eine Thrombose der Vena cava inferior ist eine sehr seltene Komplikation des stumpfen Traumas des Abdomens. Wir berichten über einen Patienten, bei dem 6 Monate nach einem primär asymptomatischen stumpfen Bauchtrauma durch große retroperitoneale Raumforderungen eine symptomatische Harnstauungsniere auftrat. Durch eine retroperitoneale Freilegung bei Verdacht auf Vorliegen von Lymphomen im Rahmen einer malignen Grunderkrankung wurde die überraschende Diagnose einer Thrombose der Vena cava inferior gestellt. Nach Ausschluß anderer Ursachen kann diese auf das stattgehabte abdominelle Trauma ursächlich zurückgeführt werden.

Abstract

Thrombosis of the inferior caval vein following blunt abdominal trauma is extremly rare. Herein we report the case of a patient who developed clinical symptoms caused by large retroperitoneal masses and hydronephrosis 6 months after blunt abdominal trauma. Retroperitoneal lymphadenectomy was performed and resulted in the surprising diagnosis of thrombosis of the inferior vena cava. There were no reasons to explain the caval thrombosis except the blunt abdominal injury.
