Gigantische Hydronephrose mit einer Masse von nahezu 25% des Körpergewichtes

Wir berichten über den Fall einer auf dem Boden einer Unterabgangsstenose entstandenen gigantischen Hydronephrose mit einem Gesamtinhalt von ca. 19 Litern, welches einem Anteil am Gesamtkörpergewicht von nahezu 25% entspricht. Die Therapie bestand in einer initialen perkutanen Nephrostomie und intermittierenden Entleerung der Hydronephrose mit anschließender Nephrektomie. Diagnostisches Vorgehen und mögliche differentialdiagnostische Schwierigkeiten werden beschrieben. Auch ohne vorausgegangenes Trauma sollte eine Hydronephrose als Ursache einer massiven abdominellen Raumforderung in Erwägung gezogen werden.

Abstract

We report on a giant hydronephrosis with a contents of ca. 19 liters that had developed at the site of a ureteropelvic junction obstruction. Therapy consisted of percutaneous nephrostomy and reduction of the hydronephrosis by successive drainage over 18 days, followed by nephrectomy. Diagnostic workup and possible pitfalls of the differential diagnosis are addressed. Even without prior trauma, hydronephrosis should be considered as one of the possible causes of massive abdominal swelling.
