Seltene Manifestation der Wegenerschen Granulomatose an der prostatischen Urethra

Zusammenfassung:

Fallbericht eines 52jährigen Patienten mit Manifestation der Wegenerschen Granulomatose (WG) an der prostatischen Urethra sowie am oberen Respirationstrakt. Die WG ist eine vaskulitische Systemerkrankung, die durch den Befall des Respirationstraktes, der Nieren sowie der Blutgefäße charakterisiert ist. Eine Beteiligung der männlichen Adnexe ist selten, so wird ein Befall der Prostata in 2-7% beschrieben. Eine Manifestation der WG an der Harnröhre ist unseres Wissens in der Literatur lediglich in 6 Fällen dokumentiert. Als Standardtherapie gilt das “Fauci-Schema”, eine immunsuppressive Kombination aus Cyclophosphamid 2 mg/kg Körpergewicht/die und Glukokortikoiden 1 mg/kg Körpergewicht/die. Entscheidend für die Prognose der WG ist die renale Beteiligung.

Abstract

Report of a 52-year-old patient with manifestation of Wegener's granulomatosis on the pars prostatica urethrae and the upper respiratory tract. Wegener's granulomatosis (WG) is a systemic disease which is characterized by involvement of the respiratory tract, kidneys and blood vessels. A location on the male adnexes is rare, and prostatic involvement in WG has consequently been estimated to amount to 2-7%. Manifestation of WG on the pars prostatica urethrae has as yet been reported in the literature, to our knowledge, in only 6 cases. Standard therapy is the “faucischeme”, an immune suppressive combination consisting of cyclophosphamide 2 mg/kg body weight and prednisone 1 mg/kg body weight. The prognosis of WG is determined by renal involvement.
