Summary

Needle biopsy is helpful in classifying pulmonary neoplasms according to the TNM-system. A population of 2726 patients is described in which needle biopsy differentiated between neoplasm and inflammatory disease. In the subgroup of patients with neoplasms (46 %) TNM-staging was established by needle biopsy. Extrapolation of these results aided in deciding whether the diagnosed lesions were operable or inoperable. Needle biopsy is also helpful in establishing the diagnosis of oat cell carcinoma. For all practical purposes this is an inoperable lesion. The patient may such be saved unnecessary thoracotomy; an alternative treatment to surgery i. e. radiotherapy and/or chemotherapy is then available. Finally, the combination of needle biopsy with subsequent TNM-staging is useful as a prognostic device.

Zusammenfassung

Die Nadelbiopsie leistet bei der Stadieneinteilung intrapulmonärer Neoplasmen nach dem TNM-System gute Dienste. Eine Population von 2726 Patienten wird beschrieben, bei welchen die Nadelbiopsie eine Differenzierung zwischen Tumorbildung und entzündlicher Erkrankung ermöglichte. Bei der Patientenuntergruppe mit Neoplasmen (46 %) wurde eine TNM-Stadieneinteilung mittels Nadelbiopsie vorgenommen. Die Extrapolierung dieser Ergebnisse half bei der Entscheidung, ob die diagnostizierten Läsionen operabel oder nicht operabel waren. Die Nadelbiopsie ist auch nützlich bei der Erstellung der Diagnose des kleinzelligen Bronchialkarzinoms. Da es sich hierbei praktisch um ein nicht operables Karzinom handelt, erspart die Nadelbiopsie dem Patienten eine unnötige diagnostische Thorakotomie und ermöglicht die Anwendung einer alternativen Therapie, z. B. Radiotherapie und/oder Chemotherapie. Schließlich erweist sich die Kombination von Nadelbiopsie mit darauffolgender TNM-Stadieneinteilung als prognostisches Hilfsmittel.
