Zusammenfassung
In einer prospektiven Studie über 59 Patienten mit akuter Pankreatitis wurde der Stellenwert der Computertomographie für die Frühprognose bestimmt. Neben drei computertomographischen Befunden (massive retroperitoneale Exsudation mit Verbreiterung des Retroperitonealraumes, intraperitoneale Spiegelbildungen und Aszites) wurden weitere sechs klinische (Ileus) und laborchemische Parameter (LDH, Blutzucker, Harnstoff-N, Kreatinin und Kalzium) sowie das Lebensalter des Patienten berücksichtigt. Die Patientengruppe mit 5 oder mehr dieser prognostisch ungünstigen Parameter, die in den ersten 24 bis 48 Stunden nach stationärer Aufnahme erhoben wurden, wies eine Letalität von ca. 70 % auf. In dieser Gruppe befanden sich über 90 % aller Patienten mit letal verlaufender Pankreatitis.
Summary
A prospective study was performed in 59 patients with acute pancreatitis to find out the value of computed tomography for an early prognosis. CT findings (retroperitoneal fluid collections with enlargement of the retroperitoneal space, intraperitoneal air-fluid levels and ascites), clinical (age, ileus) and laboratory tests (LDH, blood glucose, urea, creatinine and calcium) were taken into consideration. These data were collected during the first two days after hospitalisation and turned out to be of considerable prognostic value: About 70 % of the patients died when 5 or more of these pathological findings existed. More than 90 % of all patients who died belonged to this group.
