Zusammenfassung
Die Ultraschalluntersuchung der Parotis im grauwertgestuften real-time-Schnittbild ist eine bislang kaum praktizierte Untersuchungsmethode. So lassen sich gerade mit der hochauflösenden 5-MHz-Sonde und unter Verwendung einer Wasservorlaufstrecke weitgehende Aussagen über Form, Größe, Berandung und Konsistenz pathologischer Parotisveränderungen machen. Während die Berandung gutartiger Raumforderungen immer scharf abgrenzbar ist, sind maligne Prozesse zumeist unscharf begrenzt. Eine weitere Differenzierung erlaubt die Echotextur dieser Veränderungen. Während die normale Parotis homogen strukturiert und, ähnlich der Schilddrüse, mit einem mittleren bis helleren Grauwert zur Darstellung kommt, imponieren solide, zystische oder entzündliche Erkrankungen als grauwertunterschiedliche und/oder reflexinhomogene Strukturen. Am Beispiel von 43 Patienten wird die Sonomorphologie dieser Veränderungen dargestellt und mit den histologischen Befunden verglichen.
Summary
Ultrasound examination of the parotid by grey-scale real time is a method which has hardly been used so far. By using a high resolution 5 MHz probe and a water bag, it is possible to obtain detailed information concerning the type, extent, margins and consistency of pathological changes in the parotid. The margins of benign lesions are always clearly defined, whereas magligant processes usually have ill-defined margins. Further differentiation is possible from the type of echo. Normal parotids have a homogeneous structure, similar to the thyroid, with a medium to bright grey-scale value; solid, cystic or inflammatory lesions present irregular or inhomogeneous structures. The sonographic appearances are illustrated by the findings in 43 patients and are compared with their histology.
