Atypischer “Blasentumor” - bei Urethrastriktur in die Harnblase penetrierter iliakaler Schenkel eines aortofemoralen Bifurkations-Bypass

Zusammenfassung

Vorgestellt wird ein Patient mit einem bis ins Urothel der Harnblase penetrierten Schenkel einer Y-Gefäßprothese, der als Harnblasentumor imponierte. Nach transurethraler Probeexzision kam es zu einer fulminanten Blutung in die Blase, die nur durch Kompression der Harnröhre mit konsekutiver Blasentamponade vorübergehend beherrschbar war. Danach konnte durch offenes Vorgehen der Prothesendefekt versorgt und der Prothesenschenkel wieder nach extravesikal verlegt werden.

Abstract

We present a male patient in which the iliacal section of an Y-prosthesis had penetrated into the urothelium of the bladder wall, so that it seemed to be a tumour of the urothelium. After performing a transurethral resection of this area we had immediately extensive bleeding into the bladder, and only tamponade of the bladder following compressing the urethra stopped it temporarily. After that it was possible first to shut the leak of the prosthesis, and second to replace the prosthesis out of the bladder in the pelvic area.
