Zusammenfassung
Zur Überprüfung des Einflusses der piezoelektrischen Stoßwellengeneration auf das Reizleitungssystem des Herzens wurde bei 50 konsekutiven Patienten während der EPL-Behandlung eine permanente EKG-Ableitung aufgezeichnet. In einem experimentellen Versuchsaufbau wurde zudem ein Demandschrittmacher der Stoßwellenapplikation bzw. den hierbei entstehenden Feldstärkenänderungen ausgesetzt.
Die Versuche zeigten, daß die Sinusknotenfunktion - bzw. die kardiale Erregungsausbreitung - durch die Stoßwellengeneration nicht beeinträchtigt wird. Bei der SM-Exposition konnte jedoch eine Impuls-inhibition konstant nachgewiesen werden. Eine mechanische Zerstörung des SM trat nicht ein. Als Konsequenz muß bei Schrittmacherpatienten eine SM-Modulation mittels Magnet zur Gewährleistung einer konstanten SM-Frequenz sowie ein EKG-Monitoring gefordert werden.
Abstract
In order to investigate the influence of piezoelectric Shockwave generation on the conduction system of the heart, 50 consecutive patient underwent continuous ECG monitoring during EPL treatment. In addition, a demand pacemaker was subjected to application of shock waves and the resulting changes in field intensity in an experimental construct.
The experiments showed that the function of the sinoartrial (SA) node and the passage of the cardiac impulse are not impaired by Shockwave generation. During the exposure of the pacemaker, however, an inhibition of impulse could be shown consistently. Mechanical damage to the pacemaker did not occur. As a consequence, pacemaker modulation by means of a magnet as well as ECG monitoring must be required for patients with pacemakers in order to ensure a constant pacemaker frequency.
