Erhebung der Therapie- und Pflegebedürftigkeit von Patienten in neurologischen Rehabilitationseinrichtungen im Freistaat Bayern

Summary

To estimate the need of neurologic rehabilitation capacities, a survey was taken involving all Bavarian hospitals specialized in the rehabilitation of neurologic patients. The hospitals were asked to report diagnosis, sex, age and Barthel-Index for each patient on a fixed day. Additional aggravating facts like tracheostoma, orienting- or communication disorder or swallowing difficulties were reported. If one of these complications was found then a Barthel-index of more than 50 points was reduced to 50 points. The so calculated index was called a modified Barthel-index (BIM). The data of 22 hospitals (10 acute care hospitals covering 362 beds, 10 rehabilitation centers covering 1153 beds and 2 children's hospitals covering 67 beds) could be analyzed. 87% of the patients in acute care hospitals, 42% of patients in rehabilitation centers and 73% in children's hospitals had a BIM of less than 100, i.e. they were handicapped in some way. The data for BIM-values of 0 points (totally dependent on external help) were 14%, 2% and 28% for acute care, rehabilitation and children's hospitals, respectively. Stroke was the diagnosis mentioned most frequently with 150 in acute care hospitals, 309 in rehabilitation centers and 6 patients in children's hospitals. Severe head injuries were typically concentrated in acute care hospitals (89), less in rehabilitation centers (26) and in children's hospitals (10).

The median BIM-values for stroke patients were 50 points in the acute care group, 50 points in the rehabilitation group and 0 points in the children's group. The corresponding figures for patients with severe head injuries read 10, 50 and 15 BIM-points, respectively.

Zusammenfassung

Um eine Bedarfsabschätzung der in der neurologischen Rehabilitation notwendigen Bettenkapazitäten zu ermöglichen, wurde eine Bestandsaufnahme in allen bayerischen Kliniken, die sich auf die Rehabilitation von neurologischen Patienten spezialisiert haben, durchgeführt. Die Kliniken wurden gebeten, zu einem Stichtag Diagnose, Geschlecht, Alter und Barthel-Index mitzuteilen. Zusätzlich wurden erschwerende Faktoren wie Tracheostoma, Orientierungs- oder Verständigungsstörung und Schluckstörung mitgeteilt: Wenn eine dieser Komplikationen vorlag, wurde ein Barthel-Index von mehr als 50 Punkten auf 50 Punkte reduziert. Der so berechnete Index wird im folgenden mit BIM (modifizierter Barthel-Index) bezeichnet. Zur Auswertung kamen die Daten von 22 Kliniken (darunter 10 Akutkliniken mit 362 Betten, 10 Rehabilitationskliniken mit 1153 Betten und 2 Kinderkliniken mit 67 Betten). 87% der Patienten in Akutkliniken, 42% in Rehabilitations- und 73% in Kinderkliniken hatten einen BIM von weniger als 100, was bedeutet, dass diese Patientengruppe in irgendeiner Weise behindert war. 0 Punkte auf dem BIM (vollständig von fremder Hilfe abhängige Patienten) hatten 14% in der Akut-, 2% in der Reha- und 28% in der Kindergruppe. Schlaganfall war die am häufigsten genannte Diagnose mit 150 dieser Patienten am Stichtag in Akutkliniken, 309 in Reha-Zentren und 6 in Kinderkliniken. Patienten mit Schädel-Hirn-Traumata wurden im Gegensatz dazu überwiegend in Akutkliniken (89), weniger in Reha-Kliniken (26) oder in Kinderkliniken (10) versorgt.

Die medianen BIM-Werte für Schlaganfallpatienten waren 50 Punkte in Akutkliniken, 50 Punkte in Reha-Kliniken und 0 Punkte in den Kinderkliniken. Die entsprechenden Zahlen für Schädel-Hirn-Traumata waren 10, 50 und 15 BIM-Punkte.
