Zusammenfassung
Venöse Blutungen aus dem Plexus prostaticus sind in der initialen Phase der totalen Prostatektomie schwierig zu stillen. Der periprostatische venöse Plexus wird nicht nur durch die Vena dorsalis penis profunda, sondern auch durch Venen der unteren Extremitäten gespiesen, wie anhand von Leichen-Phlebographien demonstriert wird. Durch die kurzdauernde Sperre der Beinzirkulation mit Hilfe von pneumatischen Manschetten, sowie durch Abklemmen der Arteriae iliacae internae und durch das Anlegen eines Tourniquets um die Penisbasis, kommt es zu einer partiellen pelvinen Ischämie.
Der Effekt dieser Maßnahmen wird anhand von intraoperativen Eotografien belegt.
Die partielle pelvine Ischämie verbessert die Übersicht im Operationsgebiet und gestattet eine genaue Präparathion des Prostataapex, womit die Gefahr einer Inkontinenz verringert wird.
Abstract
Massiv bleeding from venous plexus in the deep pelvis is difficult to control during the initial phase of total prostatectomy. Anastomoses between leg veins and the prostatic plexus are demonstrated by phlebography. The interruption of venous return from the lower limbs by pressurized cuffs reduces the bleeding remarkably. The technique of the partial pelvic ischemia is described. It improves the sight of the surgeon performing a radical prostatovesiculectomy and thereby minimizes the risk of incontinence.
