Abstract
Ruptures of the stomach occur rarely after crush injuries of the abdomen. On the basis
of two of our cases in children, the peculiarities of history, diagnosis and therapy
of this clinical picture are mentioned. The injury occurs mostly when the stomach
is full. It often produces long tears, especially near the curvatures. The freshly-ingested
food is distributed over the whole abdominal cavity. Besides closing the perforation,
the aim of the operation is to remove all food-particles from the peritoneal cavity
through extensive lavage, in order to prevent the dangerous, often lethal septic complications.
Zusammenfassung
Magenrupturen nach stumpfem Bauchtrauma sind selten. Anhand zweier eigener Beobachtungen
bei Kindern wird auf die Besonderheiten der Anamnese, der Diagnostik und der Therapie
dieses Krankheitsbildes hingewiesen. Die Verletzung entsteht meist bei gefülltem Magen.
Es kommt oft zu langen Rissen meist an den Kurvaturen. Die eben aufgenommenen Speisen
werden diffus in der gesamten Peritonealhöhle verteilt. Ziel der Operation ist neben
dem Verschluß der Perforationsstelle alle Speisen aus der Peritonealhöhle zu entfernen
- ausgiebige Lavage -, um die gefährlichen, oft tödlich verlaufenden septischen Komplikationen
zu verhindern.
