Die dynamische infravesikale Obstruktion im Rahmen der benignen Prostatahyperplasie (BPH) beruht auf einer α1A-Rezeptor vermittelten Steigerung der Kontraktilität der glatten prostatischen Muskulatur. Ziel der vorliegenden Studie war es, den intrazellulären Wirkungsmechanismus der α1-Rezeptorinduzierten glattmuskulären Kontraktion sowie die Rolle von Alpharezeptorenblockern und zyklischen Nukleotiden auf zellulärer Ebene mit Hilfe der patch-clamp-Technik zu untersuchen. Phenylephrin (PE) stimulierte den transmembranären L-Typ Ca2+-Strom von 7,8 μA/cm2 bis auf 18,2 μA/cm2 und erhöhte simultan die freie zytosolische Ca2+-Konzentration [Ca2+]i bis auf 1,9 μM. Intrazelluläre Applikation von Inositol 1,4,5-Trisphosphat (IP3), imitierte die Effekte von PE, während Blocker der intrazellulären Ca2+-Freisetzung (Ryanodin (100 μM), Thapsigargin (10 nM)) den Effekt unterdrückten. Die intrazelluläre Ca2+-Liberation erscheint damit als zelluläres Korrelat der dynamischen Obstruktion im Rahmen der BPH. Die a1-Rezeptor induzierte Stimulation des L-Typ Ca2+-Stromes konnte durch Alfuzosin (IC50 0,39 ± 0,11 nM) Tamsulosin (IC50 0,52 ± 0,12 nM) > Terazosin (IC50 1,85 ± 0,32 nM) > Doxazosin (IC50 2,40 ± 0,3 nM) konzentrationsabhängig, vollständig und reversibel inhibiert werden. In Analogie dazu fanden sich an der Nierenarterie folgende IC50-Konzentrationen: Terazosin (34,5 ± 0,6 nM) > Tamsulosin (46,7 ± 3,5 nM) > Doxazosin (121,8 ± 5,4 nM) > Alfuzosin (212 ± 7,2 nM). Damit ergibt sich folgender Prostataselektivitätsscore (Nierenarterie-IC50/Prostata-IC50): Alfuzosin (543,59) > Tamsulosin (89,81) > Doxazosin (50,75) > Terazosin (18,65). Administration der zyklischen Nukleotide cAMP und cCMP inhibierten die PE induzierte Kontraktion um bis zu 85%. Die Kenntnis der zellulären Signaltransduktion der glatten, prostatischen Muskulatur ermöglicht die Entwicklung innovativer Therapieansätze.

Abstract

The dynamic infravesical obstruction occuring in BPH is based on a α1A -adrenoceptor mediated stimulation of prostatic smooth muscle contractility. The present study yields the cellular mechanism of α1A -adrenoceptor induced prostatic smooth muscle contraction and the selectivity and potency of various adrenoceptor blockers and cyclic nucleotides by using the patch-clamp technique in enzymatically isolated human prostatic myocytes. Phenylephrine (PE) stimulated the transmembranous L-type Ca2+-current from 7.8 μA/cm2 up to 18.2 μA/cm2 simultaenously increasing the free cytoplasmic Ca2+-concentration [Ca2+], up to 1.9 μM. Intracellular application of inositol 1,4,5-trisphosphate (IP3) imitated while blockers of intracellular Ca2+-release suppressed the PE mediated response. Therefore intracellular Ca2+-liberation seems to be crucial for the dynamic obstruction in BPH. The α1A-induced stimulation of Ca2+-channel current was dose-dependently and reversibly suppressed by alfuzosin (IC50 0.39 ± 0.11 nM) > tamsulosin (IC50 0.52 ± 0.12 nM) > terazosin (IC50 1.85 ± 0.32 nM) > doxazosin (IC50 2.40 ± 0.30 nM). In renal artery smooth muscle tissue the following IC50 were determined: terazosin (34.5 ± 0.6 nM) > tamsulosin (46.7 ± 3.5 nM) doxazosin (121.8 ± 5.4 nM) > alfuzosin (212 ± 7.2 nM) leading to selectivity-scores (renal artery-IC50/prostate-IC50) of alfu-zosin (543.59) tamsulosin (89.81) doxazosin (50.75) terazosin (18.65). The cyclic nucleotides cAMP and cCMP inhibited the PE contraction up to 85%. The knowledge of cellular signal transduction enables the development of innovative therapy strategies.
