Zusammenfassung:
Ziel: Die präpubische Urethrektomie hat sich als technisch einfache und komplikationsarme Alternative zur perinealen Urethrektomie erwiesen. Eine Steinschnittlagerung ist nicht erforderlich, was Operationsdauer und Thromboserisiko vermindert. Die stumpfe und teils blinde Dissektion der bulbären Harnröhre im dorsalen Anteil kann jedoch mühsam und schwierig sein. Deshalb wurde eine einfache Modifikation entwickelt. Material und Methoden: Von 1996-8/1998 wurden bei 16 Patienten, bei denen nach radikaler Zystektomie kein orthotoper Blasenersatz durchgeführt wurde, eine präpubische Urethrektomie vorgenommen. Die Operationstechnik entsprach prinzipiell der konventionellen Operationsmethode nach Van Poppel und Baert, Sie unterschied sich jedoch im letzten Schritt. Nach Erreichen der bulbären Harnröhre und Aufteilung der Corpora cavernosa in die Crura ist eine blinde und stumpfe Präparation nach proximal bis zum Erreichen der oberen Mobilisationsgrenze notwendig. Nach Zystoprostatovesikulektomie wurde nach periurethraler stumpfer Mobilisation die bulbäre, frei präparierte Harnröhre im dorsalen Anteil nach retrograder Einlage eines Katheters gestrippt und auf diese Weise vollständig entfernt. Ergebnisse: Durch die Modifikation gelang eine raschere und schnellere präpubische Urethrektomie. Die Operationszeit betrug 20-30 Minuten. Größere Nachblutungen traten nicht auf. Schlußfolgerung: Die vorgestellte Modifikation erlaubt eine Verringerung der Operationsdauer und erleichtert die größtenteils blinde und stumpfe Urethrektomie nach proximal.
Abstract
Purpose: Prepubic urethrectomy is a simple and safe alternative to perineal urethrectomy. A lithotomy position can be avoided diminishing operation time and risk of thrombosis. However, the blunt and blind dissection of the bulbar urethra can be difficult. Therefore a simple modification was developed. Materials and Methods: From 1996 to 8/1998 a prepubic urethrectomy in 16 patients with bladder cancer who required a radical cystectomy and no orthotopic bladder replacement, was performed. The operation technique corresponded to the method of Van Poppel and Baert. However, the final step was different. After cystoprostatectomy and blunt periurethral mobilization the urethra was stripped and totally removed after retrograde urethral insertion of a Foley catheter. Results: The modification of the established prepubic urethrectomy allowed a quicker and easier operation. The operation time was 20-30 minutes. Serious postoperative bleeding did not occur. Conclusion: The demonstrated modification leads to a reduction of operation time and makes the last step of proximal urethrectomy with blind and blunt dissection easier.
