Nierensegmentarterienaneurysma: Spiral-CT-Diagnostik und Therapie mittels Transkatheter-Embolisation

Einleitung: Technische Fortschritte in der Computertomographie erleichtern die Diagnose und die Therapieplanung von Nierenarterienaneurysmata. Kasuistik: Ein 77jähriger Patient wird nach einmalig aufgetretenem einseitigen Flankenschmerz in Verbindung mit Makrohämaturie mit der Verdachtsdiagnose eines Nierentumors zugewiesen. Im Rahmen der präoperativen Abklärung wird eine Spiral-CT durchgeführt. In der Phase der arteriellen Kontrastmittelanflutung zeigt sie eine intensiv anreichernde 2,5 cm große Raumforderung in der linken Niere; die Diagnose eines Nierenarterienaneurysmas wird gestellt. Eine therapeutische Angiographie mit superselektiver Coil-embolisation wird komplikationslos durchgeführt. Das Aneurysma wird hierbei vollständig verschlossen. Bei CT-Nachuntersuchungen 12 und 22 Monate nach Embolisation ist in Höhe des Aneurysmasacks größenkonstant hypervaskularisiertes Gewebe nachweisbar. Die Spiral-CT mit CT-Angiographie hat im vorliegenden Fall sämtliche für die Indikation der interventionell radiologischen Behandlung wesentlichen Informationen geliefert und so eine einzeitige Durchführung der angiographischen Intervention ermöglicht. Schlußfolgerung: Der vorgestellte Fall illustriert die diagnostischen Vorteile der Spiral-CT-Technik und die interventional radiologische Therapie bei Nierenarterienaneurysma.
Abstract
Introduction: Technical improvement of computed tomography scanning facilitates the diagnosis and treatment planning of renal artery aneurysms. Case Report: A 77-year-old male patient complained about acute flank pain and gross hematuria. During preoperative work-up, a helical CT study was performed. In the arterial phase of contrast enhancement, a lesion measuring 2.5 cm with intensive contrast filling is detected in the left kidney which permits the diagnosis of a renal artery aneurysm. Treatment consisted of superselective coil embolisation. The aneurysm was completely occluded at angiographic and computed tomographic control. The patient was followed up 12 months after embolisation and CT demonstrated contrast enhancement in the location of the aneurysmal sac. Additional angiography was obtained and ruled out reperfusion of the aneurysmal cavity. The hypervascularized residuum remained unchanged at follow-up 22 months after embolisation. Conclusions: The presented case illustrates the diagnostic advantages of spiral computed tomography and the percutaneous transcatheter treatment for a renal artery aneurysm.
