Zusammenfassung
Das Real-time-B-Scan-Verfahren ist ein neues und wesentliches Prinzip zur Diagnose von Carotisstenosen. Während die Computertomographie nur eine geringe Sensitivität zur Erfassung von Obliterationen an den Halsarterien besitzt, hat die B-Scan-Sonographie eine hohe Sensitivität und Spezifität. Sowohl bei unklarem klinischem als auch computertomographischem Befund empfiehlt es sich, vor der Angiographie der extrakraniellen Hirnarterien den B-Scan für eine strengere Indikation zur Angiographie und damit zur Vermeidung des Untersuchungs- und Strahlenrisikos zunehmend einzusetzen. Ferner eröffnet seine breite Anwendung die Chance, durch Früherkennung von Carotisobliterationen Schlaganfälle zu verhindern, da sich die Patienten früher einem gefäßchirurgischen Eingriff unterziehen können.
Abstract
The real-time-B-scan is a new and essential method for diagnosing carotid stenoses. Whereas computer-assisted tomography shows only a minor sensitivity for detection of obliterations of cervical arteries, B-scan sonography has high sensitivity and specificity. B-scans should be increasingly performed both after uncertain clinical and computer tomographic findings prior to angiography of extra-cranial cerebral vessels. A more stringent indication for angiography and thus avoidance of investigational and irradiation risk can be achieved. Wide use of B-scan sonography may, through early recognition of carotid obliterations, lead to prevention of cerebral insults as patients can undergo vascular surgery at an early stage.
