Zusammenfassung
Die Demenz mit Lewy-Körperchen ist die zweithäufigste Form der neurodegenerativen Demenz. Sie wird meist durch eine Kombination von Parkinson- und Alzheimer-Pathologie hervorgerufen, die zu einer signifikanten Beeinträchtigung der dopaminergen nigro-striatalen und der cholinergen frontobasal-kortikalen Neurotransmission führt. Die typischen Symptome bei dieser Demenz sind fluktuierende Leistungsfähigkeit, Halluzinationen, Parkinsonismus, REM-Schlaf-Störungen und Neuroleptika-Überempfindlichkeit. Bei uncharakteristischer Symptomatik kann der Nachweis einer verminderten präsynaptischen Dopamin-Transporter(DaT)-Bindung den Verdacht auf eine Demenz mit Lewy-Körperchen untermauern. Therapeutisch von Bedeutung ist es, den Nachweis einer beeinträchtigten dopaminergen Neurotransmission bei dieser Befundkonstellation auch als Hinweis auf ein ausgeprägtes cholinerges Defizit zu verstehen; beide Systeme müssen in der Therapie berücksichtigt werden.
Summary
Dementia with Lewy bodies (DLB) is the second most common form of neurodegenerative dementia. It is usually caused by a mixture of symptoms of Parkinson's and Alzheimer's disease leading to a significant impairment of nigro-striatal dopaminergic and baso-cortical cholinergic neurotransmission with typical clinical symptoms of a fluctuating course, hallucinations, parkinsonism, REM-sleep disorder and neuroleptic hypersensitivity. If the clinical presentation of DLB is uncharacteristic, the demonstration of reduced presynaptic striatal dopamine transporter (DaT) sites supports a suspicion of DLB and may lead to important therapeutic consequences. In these circumstances this evidence for compromised dopaminergic neurotransmission also indicates a significant cholinergic deficit: both require diligent therapeutic attention.
