Die Peripartum-Kardiomyopathie ist eine vital bedrohliche Herzerkrankung, die bei zuvor herzgesunden Frauen gegen Ende der Schwangerschaft oder in den Folgemonaten nach Entbindung auftritt. Eine frühzeitige Diagnose und Therapie kann die Prognose der Patientinnen verbessern. Die interdisziplinäre Zusammenarbeit von Hebammen, Pränatal- und Geburtsmedizinern, Anästhesisten, Neonatologen, Allgemeinmedizinern sowie Kardiologen ist hierfür essenziell.

Kernaussagen
  • Die PPCM ist eine seltene, aber potenziell vital bedrohliche Herzerkrankung, die gegen Ende der Schwangerschaft oder in den Folgemonaten nach Entbindung auftritt.

  • Die Symptome der PPCM sind nicht selten unspezifisch und milde Verläufe können unerkannt bleiben.

  • Bei Verdacht auf eine schwangerschaftsassoziierte Herzerkrankung ist eine umgehende kardiologische Vorstellung essenziell. Eine transthorakale Echokardiografie sowie die laborchemische Bestimmung der natriuretischen Peptide sind empfohlen.

  • Die Behandlung der akuten PPCM sollte in einem erfahrenen Zentrum mit Möglichkeiten einer komplexen intensivmedizinischen Behandlung mit Einsatz von mechanischen Kreislaufunterstützungssystemen erfolgen.

  • Neben einer medikamentösen Herzinsuffizienztherapie stellt die Behandlung mit Bromocriptin eine PPCM-spezifische Medikation dar, welche die Prognose der Patientinnen verbessert.

  • Folgeschwangerschaften sind nach vollständiger Erholung möglich, jedoch mit einem erhöhten Risiko für eine erneute bzw. weitere Verschlechterung der Herzfunktion verbunden.

  • Eine sichere und östrogenfreie Kontrazeption ist bei Patientinnen mit persistierend eingeschränkter LV-Funktion und während der Einnahme einer Herzinsuffizienzmedikation essenziell.

  • Eine interdisziplinäre Zusammenarbeit von Hebammen, Pränatal- und Geburtsmedizinern, Neonatologen, Anästhesisten, Allgemeinmedizinern sowie Kardiologen ist wichtig, um eine PPCM frühzeitig zu diagnostizieren und eine optimale Versorgung der betroffenen Frauen zu gewährleisten.
