Funktionseinschränkungen bzw. Motilitätsstörungen der Speiseröhre sollten bei der Abklärung dysphagischer Beschwerden und nicht kardialer Brustschmerzen stets in Betracht gezogen werden. Die Achalasie ist die am besten definierte primäre Motilitätsstörung der Speiseröhre – ihre 3 klinisch relevanten Subtypen werden in diesem Beitrag näher beleuchtet. Im Anschluss werden weitere klinisch relevante Motilitätsstörungen des Ösophagus vorgestellt.

Kernaussagen
  • Zur Abklärung dysphagischer Beschwerden sowie nicht kardialer Brustschmerzen sind häufig unterschiedliche Untersuchungsverfahren notwendig.

  • Die Prävalenz der Achalasie steigt an.

  • Die hochauflösende Manometrie kann verschiedene Motilitätsstörungen mit therapeutischer Relevanz unterscheiden.

  • In der Achalasie-Therapie sollte primär die gestaffelte Ballondilatation oder ein Myotomieverfahren zum Einsatz kommen. Patientencharakteristika sind bei der Therapiewahl zu berücksichtigen.

  • Die perorale endoskopische Myotomie (POEM) ist für Patienten mit Achalasie oder hyperkontraktilen Ösophagusstörungen eine gute Therapieoption.

  • Eine klinisch relevante Refluxentwicklung nach POEM ist möglich.
