Opioid- und Mehrfachabhängigkeit

Opioide sind weit verbreitet: Sie werden z. B. illegal als Droge konsumiert, zur Substitutionsbehandlung eingesetzt oder bei Schmerzzuständen auch missbräuchlich verwendet. Eine Abhängigkeit von Opioiden entwickelt sich teilweise schnell, die psychische Bindung und das Verlangen nach der Droge sind typischerweise hoch. Der klinische Verlauf ist häufig chronisch und eine Mehrfachabhängigkeit (sog. Polytoxikomanie) entwickelt sich nicht selten.

Kernaussagen
  • Opioide umfassen illegale Drogen (Heroin), Substitutionspräparate bei der Opioidabhängigkeit und Analgetika für die Behandlung von Schmerzzuständen.

  • Opioide wirken akut sedierend, anxiolytisch, hypnotisch und analgetisch.

  • Die Opioidabhängigkeit ist eine schwere Suchterkrankung und wird häufig von komorbiden psychischen Störungen und anderen Suchterkrankungen („Mehrfachabhängigkeit“) begleitet.

  • Bei der Opioidabhängigkeit stellen Opioidagonisten und psychosoziale Interventionen heute die Behandlung der Wahl dar. Zu diesem Zweck steht eine Reihe verschiedener Substanzen zur Verfügung, deren Einsatz den individuellen Gegebenheiten angepasst werden sollte.
