Zahlreiche Läsionen in der Brust sind Differenzialdiagnosen gutartiger Herd- und Tastbefunde sowie bildgebender Zufallsbefunde. Sie können benigne oder prämaligne sein, oder es handelt sich um Vorläufer- oder gar Indikatorläsionen eines Brustkrebses. Entsprechend ihrem heterogenen Erscheinungsbild ist auch die Therapie nicht einheitlich und von Begleitfaktoren abhängig.

Kernaussagen
  • In der Brust gibt es viele benigne und prämaligne Läsionen mit unsicherem biologischem Potenzial, die Differenzialdiagnosen gutartiger Herd- und Tastbefunde sowie bildgebender Zufallsbefunde sind.

  • Man unterscheidet benigne B2-Läsionen (Fibroadenom, Hamartom und UDH) von B3-Läsionen, die Risikoläsionen für das Auftreten eines ipsi- oder kontralateralen Brustkrebses sind (z. B. lobuläre intraepitheliale Neoplasie/LIN, atypische duktale Hyperplasie/ADH).

  • Weitere B3-Läsionen haben unterschiedlich hohe Risiken für das Auftreten einer höhergradig malignen Läsion in der Umgebung (z. B. radiär sklerosierende Läsionen/RS, flache epitheliale Atypie/FEA).

  • Ebenfalls gibt es obligate Vorläuferläsionen für Brustkrebs (z. B. duktales Carcinoma in situ/DCIS).

  • Nicht immer ist eine nachfolgende offene Exzision nach histologischer Sicherung nötig.

  • Um eine (operative) Über- oder Untertherapie zu vermeiden und die richtige Nachsorge zu wählen, muss das Risikopotenzial für das Auftreten eines Brustkrebses richtig eingeschätzt werden.
