Zusammenfassung

Die Wahrnehmung der Bedeutung der Funktion des rechten Herzens hat sich in den letzten Jahren in Forschung und Klinik grundlegend geändert. Im klinischen Alltag erlaubt die Echokardiografie eine rasche Beurteilung der RV-Anatomie und -Funktion (RV: rechter Ventrikel). Den Goldstandard der Bildgebung bildet die MRT, die neben einer anatomischen Darstellung des RV und einer exakten Quantifizierung der RV-Funktion auch die Charakterisierung des Myokards ermöglicht. Die Rechtsherzkatheteruntersuchung stellt einen wichtigen Bestandteil in der Diagnostik dar, erlaubt eine exakte Bewertung hämodynamischer Kenngrößen der Rechtsherzinsuffizienz und kann in besonderen klinischen Szenarien um eine Belastungsuntersuchung erweitert werden.

Abstract

The role of the right heart has changed over the last decades. Echocardiography enables a fast and comprehensive assessment of right ventricular (RV) anatomy and function in daily practice while cardiac MRI represents the gold standard in RV imaging. Right heart catheterization plays a major role in the diagnostic work-up of RV function by allowing to obtain invasive hemodynamics and, moreover, it has prognostic impact. Despite diagnostic examinations, clinical evaluation of patients is of prime importance and requires a critical assessment of diagnostic findings against the background of the clinical situation of the patients.

  • Bildgebende Diagnostik: Die Echokardiografie erlaubt eine Beurteilung der RV-Anatomie und Funktion im klinischen Alltag. Den Goldstandard bildet die MRT, welche neben einer genauen Darstellung der Anatomie, eine exakte Quantifizierung der RV-Funktion, sowie eine Charakterisierung des Myokards ermöglicht.

  • Invasive Hämodynamik: Mittels Rechtsherzkatheter ist die Erfassung von hämodynamischen Kennziffern des kleinen und großen Kreislaufs möglich, welche diagnostisch und prognostisch wertvoll sind.

  • Belastungsuntersuchungen: Abhängig von der klinischen Fragestellung provozieren verschiedene Belastungsmodalitäten unterschiedliche hämodynamische Auswirkungen. Allerdings liegen bisher wenige klinische Daten vor, sodass die Vergleichbarkeit zwischen verschiedenen Patientenkollektiven und Zentren limitiert ist.
