ZUSAMMENFASSUNG

In der Behandlung von Angststörungen gibt es sehr gute empirische Belege für die Wirksamkeit kognitiver Verhaltenstherapie, insbesondere bezgl. Expositionsbehandlung. Als pharmakologische Behandlung von Angststörungen werden SSRIs und SNRIs als Pharmakotherapie der Wahl empfohlen. Dennoch gibt es einen Anteil an Patienten, die von der gängigen Therapie nicht ausreichend profitieren oder eine Behandlung ablehnen. Im vorliegenden Beitrag wird zunächst das aktuelle psychotherapeutische Vorgehen für die einzelnen Angststörungen beschrieben. Aktuelle Forschungsfelder zur Weiterentwicklung der Psychotherapie sowie alternative verhaltenstherapeutische Therapieverfahren und Modalitäten werden anschließend erläutert. Der Einsatz von Achtsamkeit und ACT bei Angststörungen, neue Expositionsmöglichkeiten durch virtuelle Realität, internetbasierte Therapieangebote, Anregungen zur Expositionspraxis anhand des inhibitorischen Lernens, Besonderheiten in der Behandlung von Kindern und Jugendlichen sowie die Umsetzung von Kurzzeittherapieprogrammen werden vorgestellt.

ABSTRACT

Cognitive behavioral therapy (CBT), especially with exposure-based interventions, is strongly supported by empirical evidence as the first line treatment for anxiety disorders. SSRIs and SNRIs are the recommended pharmacological treatments of 1st choice. Despite these options, there remains a group of patients who do not sufficiently benefit from treatment or refuse to enter treatment. In this article, the specific CBT procedures for the individual anxiety disorders are described and scientific advances in improving treatment and developing alternative CBT strategies and modalities are presented. Further, the application of mindfulness and ACT, virtual reality exposure, internet-based therapy programs, and suggestions for the exposure practice from the inhibitory learning approach, distinctive approaches to the treatment of children and adolescents, as well as short-time therapy programs are introduced.
