Hypertrophe Kardiomyopathie

Die hypertrophe Kardiomyopathie ist die häufigste genetisch bedingte Herzerkrankung, charakterisiert durch eine ausgeprägte Hypertrophie des linken und/oder rechten Ventrikels ohne erkennbare Ursache. Klinisch bleibt die Erkrankung oft asymptomatisch. Das Risiko für den plötzlichen Herztod ist von der klinischen Symptomatik jedoch unabhängig, weshalb bei jedem Patienten eine Risikostratifizierung erfolgen sollte. Kernaussagen
  • Die HCM ist in der Mehrzahl der Fälle eine erbliche Erkrankung, sodass gründliche Familienanamnese sowie -screening erforderlich sind.

  • In 5 – 10% der Fälle können sonstige genetische Störungen, metabolische Störungen oder neuromuskuläre Erkrankungen vorliegen, die zu dem Bild einer HCM führen können. Aus diesem Grund sollte vor allen Dingen bei Vorliegen muskulärer oder neurologischer Symptome an eine HCM gedacht werden.

  • Diagnose und Behandlung der HCM bilden einen interdisziplinären Ansatz (Kardiologie, Pädiatrie, Neurologie).

  • Die Therapie der HCM verfolgt 3 Ziele: Verhinderung eines plötzlichen Herztods, Kontrolle der Beschwerden und Verhinderung thromboembolischer Komplikationen.
