Weichgewebschirurgie in der Implantologie

Weichgewebschirurgische Eingriffe und Techniken zur Optimierung der periimplantären Weichgewebe sind maßgeblich am Erfolg von Implantatversorgungen beteiligt und begünstigen das ästhetische und funktionelle Ergebnis. Im Beitrag werden verschiedene Techniken vorgestellt und anhand von Bildern illustriert.

Kernaussagen

Zur Verbesserung der Weichgewebe um Implantate gibt es eine Vielzahl an Therapiemöglichkeiten, welche in Abhängigkeit der individuellen Situation, Ästhetik und Funktion durchgeführt werden können. Zur Weichgewebsverdickung werden Bindegewebstransplantate oder Rolllappentechniken eingesetzt. Besteht ein Mangel an keratinisierter Mukosa, kann diese durch ein freies Schleimhauttransplantat augmentiert werden. Freie Schleimhauttransplantate kommen auch zum Einsatz als Socket Seal im Rahmen alveolenstabilisierender Verfahren. Gestielte Bindegewebslappen werden primär im Oberkiefer angewendet, um Alveolen plastisch zu verschließen – wie z. B. bei antiresorptiver Therapie. Xenogene Materialien können als Ergänzung oder Ersatz autologer Verfahren in Betracht gezogen werden, bringen aber hinsichtlich Langzeitstabilität gewisse Nachteile mit sich. Die Papillen sind nach wie vor eine Herausforderung in der Praxis. Eine nachträgliche chirurgische Beeinflussung ist kaum möglich. Jedoch können diverse prothetische Konzepte zur Verbesserung der Ästhetik beitragen.
