Interventionelle LV-Modulation in der Herzinsuffizienztherapie
Interventional LV Modulation in Heart FailureZusammenfassung
Trotz großer Fortschritte in der Akutversorgung des Myokardinfarkts entwickelt ein relevanter Teil der Patienten eine chronische Herzinsuffizienz. Der durch den Infarkt verursachte myokardiale Schaden kann pathologische Umbauprozesse im Herzen wie linksventrikuläre Dilatation, Veränderung der ventrikulären Geometrie und die Ausbildung von Narbengewebe initiieren, die in einer progredienten Herzinsuffizienzsymptomatik und -letalität resultieren können. Verschiedene chirurgische und interventionelle Therapieverfahren zur Modulation der pathologischen Umbauprozesse im Herzmuskel wurden entwickelt. Sie erwiesen sich als sicher und effektiv in der Reduktion linksventrikulärer Volumina, einer Steigerung der linksventrikulären Ejektionsfraktion sowie im Hinblick auf klinische Parameter. Randomisierte prospektive Studien, die einen Überlebensvorteil durch interventionelle oder chirurgische linksventrikuläre Modulation nachweisen, liegen zurzeit nicht vor.
Abstract
Despite improvements in acute care of patients, suffering from myocardial infarction, a significant portion or patients develops chronic heart failure. Myocardial injury following ischemia can initiate adverse remodeling in the myocardium, characterized by left ventricular dilation, alterations in ventricular geometry and scar formation, resulting in progressive heart failure symptoms and mortality. Various surgical and interventional therapies for modulation of adverse remodeling after myocardial infarction have been developed. They proofed to be safe and effective reducing left ventricular volumes, increasing left ventricular ejection fraction and improving clinical status. Randomized prospective Studies showing a survival benefit for an interventional or surgical left ventricular modulation are underway.

Etwa 20% der Patienten entwickeln nach Myokardinfarkt eine Herzinsuffizienz. Das Ausmaß der Dilatation des linken Ventrikels (LV) nach Myokardinfarkt bestimmt maßgeblich Herzinsuffizienzsymptome und Letalität.

Chirurgische Verfahren zur LV-Modulation zeigen in Registerdaten einen positiven Effekt auf LV-Volumina, -Ejektionsfraktion, Herzinsuffizienzsymptome. Prospektive Daten, die eine Prognoseverbesserung demonstrieren, fehlen bisher.

Interventionelle Systeme zur LV-Modulation stellen eine vielversprechende Therapieoption dar, deren Nutzen es in randomisierten Studien oder großen Registern zu belegen gilt.
