Zusammenfassung

Für telemedizinische Mitbetreuung von Patient*innen mit chronischer Herzinsuffizienz besteht Evidenz bezüglich der Senkung von Letalität und kardiovaskulärer Morbidität. Nach aktuellem Beschluss des Gemeinsamen Bundesausschusses (G-BA) wird erstmals eine digitale Methode in die Regelversorgung überführt. Hochrisikopatient*innen mit reduzierter linksventrikulärer Ejektionsfraktion haben künftig einen Leistungsanspruch, entweder mittels aktiver Implantate (ICD, CRT-P und CRT-D) oder mittels telemedizinischer Heimmessgeräte betreut zu werden. Die Indikation zum Telemonitoring stellt der/die primär behandelnde Arzt/Ärztin (PBA). Der tägliche Vitaldatentransfer erfolgt an ein kardiologisch geführtes Telemedizinzentrum (TMZ), welches den/die PBA bei auffälligen Befunden zeitnah benachrichtigt. Der/die PBA entscheidet im Einzelfall über den Bedarf einer telemedizinischen 24/7-Mitbetreuung. Ein/e kardiologische/r PBA mit einer TMZ-Infrastruktur kann die telemedizinische Mitbetreuung der eigenen Patient*innen übernehmen.

Abstract

Clinical Studies have proven the benefit of telemedicine regarding mortality and cardiovascular morbidity in chronic heart failure (HF) patients. For the first time, the Federal Joint Committee (G-BA) has approved a digital therapy method for HF as standard care in Germany. Patients with HF with reduced ejection fraction (HFrEF) are now eligible for telemonitoring with either invasive devices such as ICD, CRT-P and CRT-D or with non-invasive home measuring devices (ECG, body scale, RR, HR). Data will be transferred from the patient’s home to a cardiological telemedicine centre (TMC) that reports relevant findings to the patient’s primary care physician (PBA). The PBA may then initiate further diagnostic or therapeutic measures and decide individually about for 24/7 telemedical support. If necessary, the TMC can directly alert local emergency services. Additionally, telemedical infrastructure enables telemonitoring and telemedical treatment by the attending cardiologist without a TMC.

Was ist wichtig?
  • G-BA-Beschluss: Erstmalig wird (europaweit) eine digitale Mitbetreuungsform bei chronischer Herzinsuffizienz als Regelleistung anerkannt und künftig erstattet. Die Entwicklung und klinische Erprobung telemedizinischer Verfahren bei Herzinsuffizienz können als Beispiel für den evidenzbasierten Weg einer digitalen Innovation in die Regelversorgung gelten.

  • Translation: Der Übergang von den Bedingungen telemedizinischer Studien hin zur Anwendung in der Regelversorgung benötigt klare Prozess- und Strukturdefinitionen, die sich an den Telemonitoringstudien mit positivem primärem Endpunkt orientieren. Zu den Herausforderungen des Telemonitorings in der Regelversorgung zählt die Skalierbarkeit. Dazu bedarf es der Vernetzung der Leistungserbringer und weiterer technischer Innovationen, auch unter Nutzung von Methoden der künstlichen Intelligenz.
