Zusammenfassung
Hintergrund
Die feingewebliche Diagnostik ist zentraler Bestandteil der dermatoonkologischen Versorgung.
In den aktuellen AWMF-Leitlinien sind jedoch die Anforderungen an die dermatopathologische
Befundung uneinheitlich definiert. Dies kann zu Unklarheiten in der interdisziplinären
Kommunikation und potenziell relevanten Versorgungsunterschieden führen.
Ziel
Diese Arbeit analysiert systematisch die histopathologischen Kapitel aller derzeit
gültigen dermatoonkologischen Leitlinien im AWMF-Register. Ziel ist es, Mindeststandards,
Gemeinsamkeiten, Unterschiede und potenzielle Lücken aus dermatopathologischer Sicht
darzustellen.
Methode
Es erfolgte eine strukturierte Auswertung der Leitlinieninhalte mit Fokus auf die
Vorgaben zur Probensicherung, Befundstruktur, Diagnosesystematik, Klassifikation und
Zusatzdiagnostik. Ergänzend wurden zentrale Begriffe und Exkurse (z.B. TNM-System,
Leitlinienklassifikation) kurz erläutert.
Ergebnisse
Die Analyse zeigt eine erhebliche Heterogenität der histopathologischen Empfehlungen
– sowohl hinsichtlich Detaillierungsgrad als auch Terminologie und Praxisrelevanz.
Insbesondere bei seltenen Entitäten bestehen deutliche Definitionslücken.
Schlussfolgerung
Eine klarere und einheitlichere Formulierung der histopathologischen Leitlinieninhalte
könnte die Qualität und Konsistenz der dermatopathologischen Diagnostik verbessern
und die klinisch-pathologische Zusammenarbeit stärken.
Abstract
Background
Histopathological evaluation is a central component of dermato-oncological care. However,
current AWMF guidelines show inconsistent definitions regarding dermatopathological
reporting standards. This may lead to communication gaps and variations in diagnostic
quality.
Objective
This study systematically analyzes the histopathological content of all currently
valid dermato-oncological guidelines published in the AWMF register. The aim is to
identify minimum standards, commonalities, discrepancies, and potential omissions
from a dermatopathological perspective.
Methods
A structured evaluation of guideline chapters was conducted with focus on specimen
handling, report components, diagnostic terminology, classification systems, and ancillary
testing. Core concepts and systems (e.g., TNM, guideline types) are briefly introduced.
Results
The analysis reveals considerable heterogeneity in the histopathological recommendations
– both in terms of detail and terminology. Rare tumor entities in particular show
significant definitional gaps.
Conclusion
Clearer and more standardized histopathological recommendations within national guidelines
could enhance diagnostic consistency, facilitate interdisciplinary collaboration,
and ultimately contribute to improved patient care in dermato-oncology.
