CMC1 Joint Arthroplasty: Pearls and Pitfalls, Revisions, Problems and Solutions

Abstract

The replacement of a painful thumb carpometacarpal joint with a ball-and-socket prosthesis was introduced over 50 years ago. Continuous improvements in implant design and a surgical technique have made it a successful procedure. However, complications remain possible. This review discusses key aspects for optimising indications, surgical techniques, and treatment outcomes, as well as the management of complications when they occur. Options for revision surgery are also explored. Total arthroplasty of the thumb carpometacarpal joint represents a valuable treatment option. Knowledge of correct indications, surgical techniques, potential pitfalls, and alternative treatments is essential.

Zusammenfassung

Der Ersatz des schmerzhaften Daumensattelgelenks durch eine Kugelgelenkprothese wurde vor über 50 Jahren eingeführt. Kontinuierliche Verbesserungen im Implantatdesign und der Operationstechnik haben das Verfahren erfolgreich gemacht. Komplikationen sind jedoch möglich. In diesem Artikel werden wichtige Aspekte zur Optimierung der Indikation, der Operationstechnik und des Behandlungsergebnisses sowie das Management von Komplikationen im Falle ihres Auftretens erörtert. Auch die Möglichkeiten einer Revisionsoperation werden beleuchtet. Die totale Arthroplastik des Daumensattelgelenks ist eine wertvolle Behandlungsoption. Kenntnisse über die korrekte Indikation, die Operationstechnik, mögliche Fallstricke und alternative Behandlungsoptionen sind wichtig.
