Handgelenkarthroskopie bei entzündlich rheumatischen Erkrankungen – Was ist heute möglich?

Zusammenfassung

Die Handgelenkarthroskopie hat in den letzten Jahren durch die Veränderung des Krankheitsverlaufes entzündlich-rheumatischer Erkrankungen an Bedeutung gewonnen. Neben der Synovialektomie haben auch gelenkerhaltende Eingriffe, wie die Frakturversorgung am distalen Radius und die Reinsertion des trianguläre fibrokartilaginären Komplexes in dieser Klientel zugenommen. Die Teilarthrodesen des Handgelenkes können heute häufig arthroskopisch kontrolliert perkutan erfolgen und führen zu einer geringeren Kapselvernarbung mit geringerer postoperativer Einschränkung insbesondere der Handgelenkflexion.

Abstract

Wrist arthroscopy has gained importance in recent years due to changes in the course of inflammatory rheumatic diseases. In addition to synovectomy, joint-preserving procedures such as distal radius fracture fixation and reinsertion of the triangular fibrocartilage complex have become increasingly common in this patient population. Partial wrist arthrodesis can now often be performed percutaneously under arthroscopic guidance, resulting in less capsular scarring and less postoperative restriction of range of motion, particularly with regard to wrist flexion.
