Ein targetoides Exanthem als Zeichen des Rowell-Syndroms

Zusammenfassung

Das Rowell-Syndrom wurde erstmals 1963 beschrieben und ist gekennzeichnet durch das Vorhandensein von kokardenförmigen, erythema-multiforme-ähnlichen Läsionen, kutanem Lupus erythematodes sowie ein charakteristisches immunologisches Muster in der Serologie, wie z.B. positive gesprenkelte ANA, Anti-Ro/SSA- oder Anti-La/SSB-Antikörper und ein positiver Rheumafaktor. Weltweit wurden weniger als 100 Fälle beschrieben, was diese Erkrankung zu einer seltenen Entität macht.
Wir berichten über den Fall einer 67-jährigen Frau, bei der das Rowell-Syndrom ein Frühzeichen eines Lupus erythematodes war und einen entscheidenden Wegweiser für die Diagnosestellung darstellte. Dieser Fall unterstreicht die Notwendigkeit, erythema-multiforme-ähnliche Läsionen im Zusammenhang mit Autoimmunerkrankungen auch mit seltenen Hauterkrankungen wie dem Rowell-Syndrom in Verbindung zu bringen.

Abstract

Rowell’s syndrome was first described in 1963 and is characterized by the presence of cockade erythema multiforme-like lesions, cutaneous lupus erythematosus and a characteristic immunological pattern in the serology, such as positive speckled ANA, anti-Ro/SSA, or anti-La/SSB antibodies and a positive test for rheumatoid factor. Less than 100 cases have been described worldwide, making it a rare entity. We present the case of a 67-year-old woman, where Rowell’s syndrome was an early sign of lupus erythematosus and posed as a decisive signpost for the diagnosis. This case also emphasizes the need to link erythema multiforme-like lesions with autoimmune diseases to rare skin diseases like Rowell’s syndrome.
