Kontinuierliche Balintarbeit und Psychotherapiebedarf

Zusammenfassung

Gegenstand der Arbeit sind – basierend auf einem konkreten Fallbericht einer Balintgruppe – kritische Reflexionen zu der sinnvollen Alternative hilfreicher kontinuierlicher Balintarbeit gegenüber vorschnellen Zuweisungsbedürfnissen zum „Spezialisten“ einerseits und statistische Daten mit Bezug zur Thematik des Psychotherapie­bedarfs auf der Grundlage einer ca. 10-jährigen kontinuierlicher Balintarbeit vor Ort andererseits. Diese Analysen sind fokusiert vor allem auf Fan­tasien und Reflexionen über vermeintliche Unterversorgung / lange Wartezeiten, die bei Referenten (wie in der Fallvignette) von problematischen emotionalen Arzt-Patient-Beziehungen in der Balint­gruppe öfter thematisiert wurden. Es wurde auf der Basis einfacher Analysen (die hier aber nicht vorgestellt werden müssen und den Rahmen sprengen), wenngleich noch nicht repräsentativ, nachgewiesen, dass solche fixierte Annahmen (Unterversorgung, Wartezeiten) in dieser Form nicht unreflektiert verifizierbar sind: Nur bei ca. 25 % von 100 Personen im telefonischen Erstkontakt kam es nach methodischen Verlaufsanalysen dann auch zu einer indizierten ambulanten Psychotherapie der Patienten. Emotional-getönte Fehldeutungen und Wunschhaltungen wurden kritisch analysiert: Motivationsdefizite, Fehlerwartungen, unspezifische psychosoziale, familiäre und berufliche Krisenanliegen, die ­andere Kompetenzträger (z. B. Beratungsstellen usw.) erfordern. Diese Aspekte forcieren ein par­tiell wahrscheinlich falsches, aber immer wieder ohne repräsentative Analysen reproduziertes Gesamtbild vermeintlicher psychotherapeutischer Unterversorgung gerade auch in der Balintarbeit, weshalb abschließend klare positive Orientierungen in einem Ausblick gegeben werden.

Abstract

Matters of this paper refer – based at a profound outlined case-demonstration of a Balint-group and the positive sight and solution of the problems of this doctor-patient-relationship – to critical reflections and analyses of the demands of outpatients psychotherapy, based on nearly 10 years leading a continual working in this field. These analyses are focused to phantasies about presumed so called “under-supply” with psychotherapy resp. to long waiting time for this services, in the last time often discussed in the Balint group by the refents in their presentations of emotional problematical doctor-patient-relationship. It could be shown on the basis of methodical analyses over more years, that such fixed emotional-affected expectations or suspicions cannot be verified: So it could be shown, that only nearly 25 % of 100 persons, at first by telephone-call ­asked for outpatient-psychotherapy, could leaded and admitted to an professional indicated psychotherapy after necessarily psychodiagnostical, psychodynamic and other pre-analyses. Emotional-affected desire-attitudes and expectations would be undertaken a critical analysis: Deficits in personal motivation, illusional expectations and, at all, unespecially applications as there were psychosocial, psychoeducative, professional and / or familiar crisis-requests, a. s. o. These diffuse, unespecial, but very often urgent applications must be coped in the most times by other competence-holders and institutions (for instance special welfare, advisory and health-services, psychological counselling at differerent areas). In the past, these aspects could formed the above named false emotional-affected expectations and misattitudes, so as found in our Balint-group too. At the end of this paper it could be given an open positive look and orientations at the necessity of forced futural continual work in Balint-groups.
