Zusammenfassung

Aus der zunächst zur Gewichtsreduktion entstandenen bariatrischen Chirurgie entwickelt sich derzeit die metabolische Chirurgie. Bei Patienten mit Typ-2-Diabetes scheinen die Eingriffe am Magen-Darm-Trakt nicht nur eine effektive Methode zur Gewichtsreduktion zu sein, sondern auch das Potential zu besitzen, das mit der Adipositas einhergehende metabolische Syndrom chirurgisch zu verbessern. Auch wenn genaue Indikationen noch anhand prospektiv randomisierter Studien evaluiert werden müssen, scheint ein chirurgischer Therapie-Ansatz des Diabetes mellitus Typ 2 möglich. Diese Arbeit gibt einen Überblick über die derzeit etablierten chirurgischen Verfahren in der bariatrischen Chirurgie und zeigt mögliche Perspektiven für eine chirurgische Therapie des Diabetes mellitus Typ 2 auf.

Abstract

A new therapeutic field is developing from bariatric surgery. Gastrointestinal surgery does not only seem to be a very efficacious method for weight reduction but may also have the potential to reduce obesity-related metabolic disorders. Even if there is still a lack of prospective randomized trials evaluating the correct indications for metabolic surgery in patients with type 2 diabetes mellitus, there is the legitimate expectation that the surgical approach can be successful. This article provides anan overview of the current state of bariatric surgery and gives a surgical perspective on the treatment of type 2 diabetes mellitus.
