Begriffsbestimmungen

  • Dyspepsie: Der Begriff Dyspepsie bezeichnet keine Krankheitsentität, sondern Symptome oder einen Symptomkomplex, die auf verschiedenen Ursachen basieren können. Zu diesen chronisch oder intermittierend auftretenden Beschwerden des oberen Gastrointestinaltrakts zählen epigastrische Schmerzen, Appetitlosigkeit, Völlegefühl, Aufstoßen, Sodbrennen, Übelkeit oder Erbrechen.

  • Funktionelle Dyspepsie: Dyspeptische epigastrische Beschwerden ohne Hinweis auf strukturelle oder biochemische Anomalien (Rom-III-Kriterien).

Ätiopathogenese

  • Alterationen der Motilität, viszerale Hypersensitivität, Imbalance der autonomen Aktivität, Störungen der Interaktion von Gehirn und Verdauungstrakt sowie postinfektiöse Veränderungen werden als Mechanismen der Symptomentstehung angenommen. Diese können in Studien, aber nur selten in der Routinediagnostik nachgewiesen werden.

Diagnostik

  • Bei nicht untersuchten dyspeptischen Beschwerden ist eine initiale und zeitlich begrenzte probatorische Therapie mit Protonenpumpeninhibitoren, der Test auf H. pylori und dessen Eradikation bzw. bei Alarmsymptomen eine Endoskopie möglich.

  • Ergeben Anamnese und erfolgte Diagnostik keinen Hinweis auf eine strukturelle Erkrankung, so sollte anhand der Rom-III-Kriterien die Diagnose „funktionelle Dyspepsie” gestellt und ggf. zugrunde liegende Funktionsstörungen evaluiert werden.

Therapie

  • Abhängig von der Schwere der Symptomatik und dem Krankheitsverlauf kann die Therapie – neben der allgemeinen ärztlichen Beratung und an Triggerfaktoren orientierten diätetischen Empfehlungen – symptomorientierte medikamentöse Maßnahmen und psychosoziale Interventionen umfassen.

  • Zur rationalen symptomatischen Pharmakotherapie kommen insbesondere Substanzen zur Modulation der gastralen Motilität und Sekretion sowie der viszeralen Nozizeption in Betracht.
