Rehabilitation nach Amputation an der unteren Extremität – eine Herausforderung für das interdisziplinäre Team

Zusammenfassung

Die Rehabilitation nach einer Amputation an der unteren Extremität ist eine der am meisten fordernden Aufgaben in der Rehabilitationsmedizin. Bei etwa 60 000 Amputationen jährlich in Deutschland entfällt ein nicht unwesentlicher Anteil immer noch auf die sog. „Major-Amputationen“, d. h. transfemorale und transtibiale Amputationshöhen, selten Knieexartikulationen. Überwiegend betrifft dies ältere Patienten, die unter Durchblutungsstörungen und / oder Diabetes mellitus leiden. Die Patienten sind in der Regel multimorbide und weisen zusätzlich erhebliche Begleiterkrankungen und Risikofaktoren auf.
Die Rehabilitation beginnt bei diesen Patienten bereits präoperativ, nämlich einerseits in der Vorbereitung auf den Eingriff, andererseits aber auch mit der richtigen Wahl der Amputationshöhe, und anschließend – dies ist entscheidend für den weiteren Rehabilitationsverlauf – in der korrekten chirurgischen Durchführung, speziell der Vermeidung von Amputationsneuromen in belasteten Prothesenbereichen. Hauptziele sollten die möglichst periphere Amputationshöhe und sichere Wundheilung sein. Die Patienten sollten aufgrund des postoperativen Schwellungszustands vorzugsweise ohne Prothese in die Rehabilitation verlegt werden. Dort schließt sich ein intensives, interdisziplinäres, multimodales Rehabilitationsprogramm an. Jedes Teammitglied ist wichtig. Im Zentrum stehen der Patient und seine Rehabilitationsziele. In der Frühphase steht speziell die Ausschwemmungstherapie im Vordergrund, die Kräftigung der oberen Extremität im Hinblick auf Stützkraftverbesserung sowie anschließend die Versorgung mit einer Interimsprothese, mit der der Patient im Barren bis zur Gangsicherung mobilisiert wird. Erst dann darf der Patient ohne Barren laufen. Die Auswahl der Prothesenpassteile kann in der Rehabilitation im Team anhand der prognostischen Funktionsklassen wesentlich besser erarbeitet werden als anderswo. Die Prognose nach einer suffizient durchgeführten Rehabilitation zeigt, dass auch bei hoch betagten Patienten erfolgreiche Prothesenversorgungen in mehr als 80 % der Fälle möglich sind.

Abstract

Rehabilitation after an amputation at the lower extremity is one of the most demanding tasks in rehabilitative medicine. From approximately 60 000 amputations performed annually in Germany, a considerable number lies within the so-called “major amputations” i. e., transfemoral and transtibial but seldom knee extraction. This mainly affects older patients suffering from circulatory disturbance and / or Diabetes mellitus. The patients are usually multimorbid and portray several additional concomitant diseases and risk factors. 
The rehabilitation for these patients begins in the pre-operative stage, where they are prepared for the operation as well as the choice of the correct amputation height and conclusively, the correct surgical operation involving the avoidance of amputation neuroma in the affected prothetic area, which is decisive for further rehabilitation procedure.
The main objective is supposed to be the realization of the most peripheral amputation height and safe wound healing. Due to the post-operative swelling condition, the patient should be brought to the rehabilitation facility without prosthesis. This is accompanied by an intensive inter-disciplinary and multi-modal rehabilitation program in which every team member plays an important role and the patients as well as the rehabilitation objectives are the focal point.
In the early stage, special water release therapy is put in the foreground, as well as the strengthening of the upper extremity in view of the improvement of the buttress power and lastly the provision of interim prosthesis with which the patient gets mobilized in parallel bars until movement becomes secure.
It is only after this that the patient walks without parallel bars. The choice of the prosthesis fitting parts can be acquired more appropriately in the team during rehabilitation based on the prognostic functional classes. The prognosis after a sufficiently carried out rehabilitation reveals, also for patients who are advanced in age, that successful prosthesis care is possible in more than 80 % of the cases.
