Zusammenfassung

Myelodysplastische Syndrome (MDS) sind eine heterogene Erkrankungsgruppe bei Patienten im fortgeschrittenen Lebensalter. Sie zeichnen sich durch eine oder mehrere Zytopenien im peripheren Blut, eine dysplastische Hämatopoese im Knochenmark sowie ein erhöhtes Risiko für die Entwicklung einer akuten myeloischen Leukämie (AML) aus. Die allogene Stammzelltransplantation ist die einzige kurative Behandlungsoption, allerdings kommen nur wenige der älteren Patienten dafür in Frage. Die Mehrheit der MDS-Patienten wird im Verlauf der Erkrankung auf Transfusionen angewiesen sein. Dabei haben insbesondere Patienten mit Niedrig-Risiko-MDS durch wiederholte Transfusionen über einen langen Zeitraum ein hohes Risiko für die Entwicklung einer Eisenüberladung, weshalb eine konsequente Eisenchelation ein wichtiger Bestandteil der supportiven Therapie ist. In den letzten Jahren konnte für verschiedene Substanzen eine Wirksamkeit in der Behandlung der MDS gezeigt werden. Im Falle von 5-Azacytidin führte dies zur Zulassung der Substanz für die Therapie der Hoch-Risiko-MDS. Bei Niedrig-Risiko-MDS hat Lenalidomid eine hohe Effektivität bei Patienten mit Deletion 5q gezeigt und führt bei vielen Patienten zu einer Transfusionsfreiheit.

Abstract

Myelodysplasic syndromes (MDS) comprise a heterogenous group of hematopoietic disorders mainly of the elderly. MDS are characterized by peripheral cytopenias, dysplastic hematopoiesis and increased risk of evolution to acute myeloid leukemia (AML). Allogeneic bone marrow transplantation is the only curative therapeutic option, but only few of the older patients are candidates for this approach. Most MDS patients will depend on regular blood transfusions and especially patients with low-risk MDS are at high risk of developing iron overload due to the longer course of their disease. Therefore, consequent iron chelation therapy is an important element of supportive care in these patients. Through the last years several drugs have shown a remarkable activity in the treatment of MDS. 5-azacytidine has been approved for the treatment of high risk MDS patients. Low-risk MDS patients that carry a cytogenetic deletion 5q show high rates of transfusion independency when treated with lenalidomide.
