Zusammenfassung

Die kieferorthopädische Korrektur eines maxillären Defizites ohne chirurgische Intervention hat meist protrudierte Schneidezähne zur Folge. Die vorgestellte „Kucher-Mechanik“ verlagert die applizierte Kraft in das Widerstandszentrum der Zähne und soll so zu einer translatorischen Labialbewegung der Schneidezähne führen. Im vorliegenden Fallbericht wird eine Patientin mit einer reduzierten Stützzone im Oberkiefer vorgestellt. Um eine unkontrollierte Kippung zu vermeiden entschied man sich für die Behandlung mit einer Kucher-Mechanik. Die gesamte Behandlungsdauer betrug 2 Jahre, wobei die Kucher-Mechanik für einen Zeitraum von 8 Wochen aktiv war. Durch Überlagerung der Fernröntgenaufnahmen konnte die Bewegung der Oberkieferschneidezähne veranschaulicht werden. Das Ergebnis der Behandlung resultierte in einer kontrollierten Kippung der Schneidezähne nach anterior. Die Patientin wurde 10 Jahre und 7 Monate nach Behandlungsende wieder vorstellig und zeigte ein stabiles Ergebnis bei Front- Eckzahnführung.

Abstract

Orthodontic correction of a maxillary deficiency without surgical intervention usually results in a protrusion of the incisors. With the so-called Kucher mechanic force is exerted in the center of resistance of the incisors, leading to a translatory labial movement. The case report presents a patient with lack of space for the upper permanent canines. The total treatment time accounted for 2 years. The Kucher mechanic was active for a period of 8 weeks. Movement of the maxillary incisors is visualized by superimposition of the cephalograms. Treatment resulted in a controlled tipping of the maxillary incisors. After 10 years in retention, a stable result has been maintained.
