Zusammenfassung

In den letzten Jahren wurden die skelettale Verankerungshilfen (TADs, Temporary anchorage devices) in Distalisationsmechaniken integriert, um den Verankerungsverlust der anterioren Dentition zu verhindern. Jedoch haben Mini-Implantate im Alveolarfortsatz eine hohe Verlustrate. Des Weiteren behindern TAD’s zwischen den Zahnwurzeln die Zahnbewegung nach distal.

Das Prinzip und das klinische Procedere beim Einsatz der Beneslider Mechanik mit einem oder zwei Mini-Implantaten im anterioren Gaumen werden dargestellt. Hierbei dient die Verwendung von Abutments der stabilen Kopplung der Apparatur mit den Mini-Implantaten.

Verglichen mit dem Alveolarfortsatz ist der anteriore Gaumen die bessere Insertionsregion mit der geringeren Verlustrate. Wegen der Gleitmechanik konnte eine körperliche Molarenbewegung erreicht werden. Das Finishing nach Distalisierung kann mit bukkalen oder lingualen Brackets, in manchen Fällen auch mit Alignern durchgeführt werden.

Abstract

To eliminate anchorage loss of the anterior dentition, skeletal anchorage devices (TADs, Temporary anchorage devices) have been integrated into distalization appliances over the last couple of years. However, failure rates of mini-implants in the alveolar process seem to be unsatisfactory. Secondly, TAD’s in between the roots may impair distal movement of the teeth.

The rationale and the clinical procedure using the Beneslider mechanics with one or two mini-implants in the anterior palate are described. A stable coupling of the TAD’s and the appliance can be achieved by usage of mini-implants with abutments.

Compared with the alveolar process the anterior palate proved to be the superior insertion site with a much lower failure rate of mini-implants. The application of a sliding mechanics results in a bodily molar distalization. After distalization, the finishing can be performed with buccal or lingual braces, in some cases with aligners.
