Zusammenfassung

Die Retention von Oberkiefereckzähnen stellt den Kieferorthopäden häufig vor komplexe diag­nostische Fragen und therapeutische Entscheidungen. Zum einen ist zu klären, wie ausreichende Platzverhältnisse für die verlagerten Zähne geschaffen werden sollen. Danach muss, abgesehen von der diffizilen Mechanik während der orthodontischen Einreihung, auch die chirurgische Freilegung und die intraoperative Säureätztechnik beim Kleben eines Attachments äußerst sachgemäß durchgeführt werden, damit es nicht zu iatrogenen Folgeschäden kommt. Anhand des vorliegenden Falls werden grundsätzliche Fragen bei der Planung dieser höchst anspruchsvollen Behandlungen erörtert, Fehler aufgezeigt und Problemlösungen diskutiert.

Abstract

Orthodontic treatment of patients with impacted maxillary canines contains complex diagnostic questions and therapeutic decisions which do not only relate to a lack of adequate space or meticulous mechanical problems. A crucial procedure is the surgical exposure and the acid-etching procedure of the impacted teeth which can lead to severe iatrogenic sequelae, if the criteria of an appropriate technique are not entirely respected. The present case report highlights the basic principles of treatment planning, shows potential mistakes and discusses problem solving methods.
