I am privileged to be the guest editor and contributor for this special focus series on extensor mechanism disruptions. Often overlooked in meetings and underrepresented in the literature, these injuries lie in a gray area that includes sports, trauma, and arthroplasty. The goal of this issue is to bring the reader up to date on contemporary management of these disabling injuries.

Dr. Della Rocca provides an excellent review on displaced patella fractures. His article also provides a concise rationale for current methods of treating displaced patella fractures. Dr. Hassan Mir contributes a great article on management of quadriceps tendon and patella tendon ruptures. Also focusing on surgical management, the reader will glean important pearls on successful management of these injuries. Dr. Adam Sassoon contributes a timely paper on management of periprosthetic patella fractures. Rarely written about, these fractures can be some of the most challenging to deal with surgically and can lead to some of the most devastating outcomes. My contribution is a manuscript on complications after patella fractures. This will provide the reader with a framework for understanding and managing complications that arise from the surgical treatment of patella fractures.

It is my sincere hope that after reading these four manuscripts, the reader will have a clear understanding of the contemporary management of these injuries. These manuscripts provide the foundation for understanding and appropriate management in an effort to advance the outcomes of these disabling injuries.
