Die atypische Depression

Die atypische Depression wird als ein Subtypus der Depression diskutiert, welcher bereits seit den 1960er-Jahren zunächst wegen der besseren Response auf irreversible MAO-Hemmer gegenüber trizyklischen Antidepressiva beschrieben wurde. Die AD ist gekennzeichnet durch das A-Kriterium der affektiven Reagibilität und die B-Kriterien der Hyperphagie mit Gewichtszunahme, Hypersomnie, einer bleiernen Schwere in den Extremitäten sowie einer Überempfindlichkeit gegenüber Zurückweisungen. Genetische Untersuchungen unterstützen die Annahme eines eigenen Subtypus der Depression. Zu den wichtigsten neurobiologischen Charakteristika der atypischen Depression gehört die reduzierte Aktivität der HHN-Achse mit einem Hypokortisolismus.

Die valide klinische Charakterisierung dieses Subtypus ist weiterhin Gegenstand der Diskussion, was insbesondere die Frage nach der Notwendigkeit des Einschlusses des A-Kriteriums der affektiven Reagibilität betrifft. Eine stärkere Betonung der vegetativen B-Kriterien und der Einschluss der Chronizität und des frühen Erkrankungsbeginns werden derzeit diskutiert.

Wegen der gleichen Wirksamkeit und des besseren Nebenwirkungsprofils wird die Therapie mit SSRI gegenüber dem Tranylcypromin bevorzugt. Evidenzen für die Gabe von Bupropion und Venlafaxin bedürfen noch weiterer Studien. Die psychotherapeutische Behandlung der Patienten mit Methoden der kognitiven Verhaltenstherapie erscheint aussichtsreich und sollte Teil eines Behandlungsplans sein.
