Rechtsmedizinische Aspekte der Kausalkette Thrombose – Lungenembolie – Tod

Zusammenfassung

Die Lungenembolie gehört zu den häufig verkannten oder falsch diagnostizierten Todesursachen, sie kann das finale Ereignis beim natürlichen und beim nichtnatürlichen Tod sein. Selbst wenn sie richtig diagnostiziert wird, erfolgt durch den leichenschauenden Arzt in einer nicht zu unterschätzenden Anzahl der Fälle eine falsche Einstufung der Todesart. Viele Kliniker übersehen die Kausalkette Geschehen – behandlungsbedingte Ruhigstellung – Thrombose – Lungenembolie. Die Lungenembolie darf nicht als eigenständiges Krankheitsbild verstanden werden, stets besteht ein pathogenetischer Hintergrund, der erkundet und natürlich auch im Leichenschauschein angegeben werden muß.

Summary

Pulmonary thrombembolism is a frequently misjudged and uncorrectly diagnosed cause of death, it may be final event in natural and abnormal death. Even if the event is correctly diagnosed the post-mortem examiners frequently certify natural death in cases of abnormal death. A lot of medical doctors fail to notice the causal chain of event, therapy caused relaxation, thrombosis, and pulmonary thromboembolism. The pulmonary thrombembolism may not be understood as independent syndrome. The pathogenetic background has to be explored and to be certified in the death certificate.

Résumé

L’embolie pulmonaire fait partie des causes de décès souvent méconnues ou mal diagnostiquées. Elle peut être l’événement final d’une mort naturelle ou non naturelle. Même lorsqu’elle est correctement diagnostiquée, dans un nombre de cas non négligeable le médecin légiste effectue un mauvais classement de la nature du décès. De nombreux cliniciens ne voient pas la chaîne causale événement, mise au repos due au traitement, thrombose, embolie pulmonaire. L’embolie pulmonaire ne doit pas être comprise comme un signe clinique en soi. Il existe toujours un contexte pathogénétique qui doit être recherché et bien entendu être mentionné sur l’acte dressé par le médecin légiste.
