Zusammenfassung

Im Zeitraum von September 1997 bis April 1999 wurde bei 429 Patienten mit einer Beinvenenthrombose in 96 Fällen eine isolierte Muskelvenenthrombose der Wade mittels Ultraschall festgestellt (22%). Die Wadenmuskelvenenthrombose war 64mal in den Gastrocnemiusvenen, 29mal in den Soleusvenen und dreimal in beiden Venengruppen lokalisiert. Bei 13 Patienten dehnte sich die Thrombose von den Gastrocnemius- bzw. Soleusvenen bis in die V. poplitea aus. Eine Wadenmuskelvenenthrombose trat bei Patienten mit einer Varikophlebitis im Unterschenkel, nach einer Immobilisation, bei einem PTS, aber auch nach Varizenoperationen oder Verödungen auf. Sie fällt durch einen Wadenschmerz beim Gehen auf und kann mit einer Kompressionssonographie sicher und schnell nachgewiesen werden. Wegen der Gefahr einer tiefen Beinvenenthrombose sollte eine isolierte Muskelvenenthrombose mittels Mobilisation, Kompression und Antikoagulation behandelt werden. Eine sonographische Verlaufskontrolle wird empfohlen.

Summary

Between September 1997 and April 1999, 96 patients with isolated thrombosis of the calf muscle veins were identifed by ultrasound out of 429 cases with deep vein thrombosis of the leg (22%). The muscle vein thrombosis was localized in the gastrocnemial veins in 64 limbs, in the soleal veins in 29 limbs and in both groups in 3 limbs. We found 13 patients with thrombus in the popliteal vein, originating from gastrocnemial or soleal vein thrombosis. Isolated muscle vein thrombosis occured in cases with thrombophlebitis, immobilization or chronic venous insufficiency. A large number was observed following either surgical therapy of varicosis or phlebosclerosation. Patients with thrombosis of the muscle veins often have pain in the calf while walking. Muscle vein thrombosis can be detected by compression sonography exactly and fast. Due to the risk of deep vein thrombosis, isolated muscle vein thrombosis of the calf should be treated by mobilization, compression therapy and anticoagulation. A follow-up control by compression sonography is recommended.

Résumé

Durant la période de septembre 1997 á avril 1999, nous avons découvert grâce aux ultrasons parmi 429 patients atteints de thrombose de la jambe 96 cas de thrombose isolée des veines du mollet (22%). Les thromboses veineuses étaient localisées dans les veines des muscles jumeaux dans 64 cas, dans les veines du muscle soléaire dans 29 cas, et simultanement dans les veines de 2 groupes de muscles dans 3 cas. Chez 13 patients la thrombose s´étendait de la veine des muscles jumeaux ou de la veine du muscle soléaire jusqu´á la veine poplitée. Une thrombose isolée des veines du mollet a été retrouvée chez des patients présentant une thrombophlébite de la jambe, une insuffisance veineuse chronique, ainsi qu´au décours d´une immobilisation, d´une sclérose ou encore d´une cure chirurgicale de varices. Elle se manifeste par une douleur dans le mollet á la marche et peut être détectée rapidement et avec précision á l´aide d´une sonographie sous compression. A cause du risque de thrombose veineuse profonde, la thrombose veineuse isolée des veines du mollet se doit d´être traitée par une immobilisation avec compression ainsi qu´une anticoagulation. Un contrôle sonographique de l´évolution de la thrombose est recommandé.
