Abstract

Summary

The aim of this work was to apply the cultured autologous keratinocytes in fibrin glue suspension in treatment of chronic non-healing venous leg ulcers. 32 patients with chronic leg ulcers caused by venous insufficiency were treated. The surface of ulcers covered 14.2 cm² to 48.6 cm². The duration of leg ulcer was between 4 to 20 years. The skin specimen 1.0 × 1.0 cm was taken from the buttock of each patient, from which keratinocytes were obtained. Next those cells were cultured. After 7 days the keratinocytes in fibrin glue were applied to the wound bed. After 2 weeks of treatment the ulceration of 13 patients healed completely whereas the ulcer formation of the rest of the patients diminished by 30%. In all patients the process of healing was successfully completed after further 4 weeks. Our method shows that a rapid healing chronic venous leg ulcer can be achieved by keratinocytes in fibrin glue suspension.

Zusammenfassung

Ziel dieser Arbeit war es, gezüchtete autologe Keratinozyten in Fibrinkleber-Suspension zur Therapie des chronischen, nicht heilenden Ulcus cruris venosum einzusetzen. 32 Patienten mit chronischen Ulcera curis aufgrund venöser Insuffizienz wurden behandelt. Die Oberfläche der Ulzera betrug zwischen 14,2 cm² und 48,6 cm². Die Ulzera bestanden seit 4 bis 20 Jahren. Das Hautstück von 1,0 × 1,0 cm, aus dem die Keratinozyten gewonnen wurden, wurde vom Gesäß der Patienten entnommen. Nachfolgend wurden diese Zellen gezüchtet. Nach 7 Tagen wurden die Keratinozyten in Fibrinkleber auf das Wundbett aufgebracht. Nach zweiwöchiger Therapie waren die Ulzera bei 13 Patienten vollständig abgeheilt, während die Ulzera bei den restlichen Patienten um 30% reduziert waren. Der Heilungsprozess war bei allen Patienten nach weiteren 4 Wochen erfolgreich abgeschlossen. Unsere Methode zeigt, dass eine rasche Heilung chronischer Ulcera cruris venosi durch Keratinozyten in Fibrinkleber-Suspension erreicht werden kann.

Résumé

Ce travail s’est fixé pour but d’étudier les effets du traitement d’ulcères veineux de jambes, chroniques et torpides, par une application de kératinocytes autologues cultivés en microgoutelettes dans une suspension de colle de fibrine. Trente-deux patients porteurs d’ulcères chroniques des jambes dus à une insuffisance veineuse ont été traités. La surface des ulcères couvrait de 14,2 cm² à 48,6 cm². Ces ulcères dataient de 4 à 20 ans. Chacun des patients s’est vu prélever un échantillon de peau d’une surface de 1,0 cm × 1,0 cm au niveau d’une fesse, en vue de récolter des kératinocytes. Ces derniers ont ensuite été mis en culture, puis appliqués en suspension dans de la colle de fibrine sur le lit de la plaie, 7 jours plus tard. Deux semaines après ce traitement, l’ulcération avait complètement guéri chez 13 patients et sa surface avait diminué de 30% chez l’ensemble des autres malades. La guérison a été totale chez tous les patients après 4 semaines supplémentaires. Notre méthode montre donc qu’on peut obtenir une guérison rapide des ulcères veineux chroniques de jambe en utilisant une suspension de kératinocytes dans une suspension de colle de fibrine.
