Zusammenfassung
Wir berichten von einem 13-jährigen Jungen mit Ulcus ventriculi-Blutung. Der Patient
wurde wegen akuten Bluterbrechens, jedoch in stabilem Allgemeinzustand vorgestellt.
Endoskopisch imponierte ein florides Ulkus am Angulus ventriculi mit einer Sickerblutung.
Anamnestisch ließ sich während des stationären Verlaufs eruieren, dass der Junge am
Vortag eine Überdosis Bromhexin-Saft getrunken hatte. Dies führte bei möglicherweise
vorgereizter Schleimhaut zu einer akuten Exazerbation mit nachfolgender Ulkusblutung.
In der psychologischen Diagnostik zeigten sich als mögliche Ursache für die Intoxikation
parasuizidale Tendenzen.
Summary
We report the case of a thirteen-year-old boy with a bleeding gastric ulcer. We saw
the patient after an acute haematemesis in a stable condition. An advanced ulcer with
an oozing haemorrhage was endoscopically identified at the gastric angle. Following
investigations showed that the patient had ingested an overdose of cough medicine
containing bromhexine the day before. This and a possibly latent gastritis caused
the acute ulcer and the following gastric bleeding. The psychological diagnostic process
revealed parasuicidal tendencies as the reason of the intoxication.
