Zusammenfassung
Obwohl die Hand anatomisch ein kleines Gebiet umfasst, unterscheidet sich die Konstruktion
der einzelnen Gelenke so, dass die intraartikulären Injektionstechniken diese Unterschiede
im Aufbau berücksichtigen müssen. Die Polyarthrose der Hände betrifft die Fingergelenke
und das für die Menschheit so wichtige Daumensattelgelenk; bei der rheumatoiden Arthritis
sind die Grundgelenke II und III krankheitsspezifisch am häufigsten betroffen. Gelingt
es durch die Injektion in diese Gelenke, sei es mit Hyaluronsäure oder mit Depot-Kortikoiden
(schonendste Synovioorthese) die Krankheitssymptome zu lindern, so können systemisch
wirkende Medikamente massiv eingespart wer-den und die Teilhabe der Patienten wird
deutlich verbessert.
Summary
Although the hand is a small area from an anatomical viewpoint, the construction of
the individual joints of the fingers must be taken into account in association with
in-traarticular injections. Osteoarthritis of the hands affects the joints of the
fingers as well as the thumb, a joint that is of extreme importance for humans. In
rheumatoid arthritis, the CMC II/III-joints are the most commonly affected joints.
If a successful injection in these joints alleviates the symp-tons of the disease,
albeit with hyaluronic acid or long acting steroids (the least aggressive synoviorthesis),
it could result in a massive reduction of systemic drugs and considerably improve
patients' daily activity.
