Zusammenfassung

Die Migräne und andere chronisch verlaufende Kopfschmerzleiden weisen verschiedene Komorbiditäten auf. Bei Migränepatienten werden überzufällig häufig unspezifische kleine Läsionen im Marklager, ischämische Läsionen in der hinteren Strombahn oder ein persistierendes Foramen ovale nachgewiesen. Der pathophysiologische Stellenwert dieser Befunde ist bislang nicht geklärt. Für die Diagnostik oder Therapie von Migränepatienten ergeben sich hieraus keine Konsequenzen. Es gibt dagegen gute Evidenz dafür, dass Angsterkrankungen, Depressionen und möglicherweise auch Persönlichkeitsstörungen nicht nur gehäuft bei chronischen Kopfschmerzen auftreten, sondern auch bei deren Chronifizierung eine Schlüsselrolle spielen. Erste vorläufige Daten aus der Integrierten Versorgung Kopfschmerz Berlin geben einen Einblick in deren Versorgungsstrukturen und den Outcome einer multimodalen Therapie chronischer Kopfschmerzpatienten.
