First clinical experience with the new cochlear implant HiFokus SlimJ from Advanced Bionics

    Introduction:

    In the care of patients with residual hearing with cochlear implants, there is a special claim to the electrodes. Advanced Bionics has developed the HiFokus SlimJ, a lateral-wall electrode designed especially for patients with residual hearing. Here we report the first 10 patients who were treated with this electrode in our clinic.

    Patients & Method:

    The HiFokus SlimJ electrode is a lateral wall electrode with a maximum insertion depth of 23 mm. The diameter of the electrode increases from 0.55 × 0.25 mm at the electrode tip to 0.76 × 0.55 mm at the basal contact. In our clinic 10 patients with residual hearing were supplied with the HiFokus SlimJ Electrode. In the DVT, the individual length of the outer wall of the cochlea was measured preoperatively and the recommended insertion depth of the electrode was estimated taking into account the hearing loss. Intraoperatively, a high dose of steroids was given intravenously. The insertion of the electrodes took place in all patients via the round window. During the insertion, the threshold of cochlear microphonics (CM) was derived via the implant and returned to the surgeon as real-time feedback. Postoperative situation assessments by means of DVT and hearing tests on the following day as well as initial adaptation.

    Results:

    The operations proceeded without complications. The situation in the Scala Tympani could be documented by means of imaging. Postoperative hearing loss, with one exception, was less than 30dB both postoperatively and initially.

    Conclusion:

    The HiFokus SlimJ electrode is easy to handle for the surgeon, in particular the feedback via the CM measurement via the implant has proven to be positive. The rate of maintenance of the residual hearing is promising.
