Synchronous unilateral pleomorphic adenoma and cystadenolymphoma of the parotid gland: two case reports and review of the literature
Introduction:
Synchronous unilateral tumors in the major salivary glands are presented in less than 0,3% of all salivary gland neoplasms. Mostly cystadenolymphomas occur multifocal in unilateral parotid glands. B-mode ultrasound, magnetic resonance imaging and fine-needle aspiration cytology are reliable preoperative diagnostic tools to differentiate between benign and malignant masses. Therapy of choice is a total parotidectomy. The aim of the study is to increase awareness of synchronous unilateral tumors in salivary glands and to review the literature.
Case Report:
We report about two patients, one 71-year-old and the other 68-year-old, each with a slowly enlarging, good movable and indolent swelling of the right parotid gland over some years.
In the first case sonographically two tumors were found in the caudal pole of the right parotid gland. The first one measured about 1,4 cm x 1,5 cm x 2,1 cm and showed cystic areas whereas the second one was about 3,2 cm x 1,5 cm x 3,1 cm and was more lobulated.
Even in the second case sonographically there appeared two tumors with 1,2 cm x 0,6 cm x 0,8 cm and 1,9 cm x 1,1 cm x 1,2 cm preauricular and in the caudal parotid pole.
In both cases a cystadenolyphoma and a pleomorphic adenoma were detected. A total parotidectomxy was performed.
Conclusion:
Mostly multifocal tumors are diagnosed as cystadenolymphomas also known as Warthin's tumors. Clinicians should be aware of coexistence of tumors of different histologic types in ipsilateral salivary glands. In most cases two benign lesions are found but in some cases benign and malignant lesions occur simultaneously.
