Castleman's disease – a case series

    Introduction:

    The Castleman's disease is a very rare heterogeneous group of lymph node tumors with potentially severe progression. The disease occurs regardless of age and gender. Two types can be differentiated: the clearly more common unicentric type and the multicentric type.

    The cervical lymphadenopathy often progresses in an asymptomatic way. Thus, this lymphoproliferative disorder is frequently diagnosed and treated by ENT specialist.

    Material and methods:

    From 2017 – 2018, the initial diagnosis of Castleman's disease was made in 3 patients at the ENT clinic of Pirna. The type of disease, the symptoms as well as the diagnostic and therapeutic algorithms are described.

    Results:

    3 patients (1 male, 2 female) between the ages of 46 and 64 were identified. Symptoms duration was inhomogeneous and ranged between 3 and 12 months. After surgical therapy the diagnosis of an unicentric hyalin-vascular type of Castleman's disease was made in 2 cases, in 1 case a multicentric plasma cell type was identified. The surgical therapy of lymph node exstirpation was in all cases curative therapy without additional drug therapy. A recurrence did not occur during the follow-up period.

    Conclusion:

    Cervical lymphadenopathy is a common complaint in ENT practice. If the disease remains intreatable without any specific discomforts or laboratory findings, a Castleman's disease should be taken into account and a rapid diagnosis by lymph node biopsy and corresponding staging is indicated.
